Sinopsis

The science of human nutrition deals with all the effects on people of any component found in food. This starts with the physiological and biochemical processes involved in nourishment—how substances in food provide energy or are converted into body tissues, and the diseases that result from insufficiency or excess of essential nutrients (malnutrition). The role of food components in the development of chronic degenerative disease like coronary heart disease, cancers, dental caries, etc., are major targets of research activity nowadays. The scope of nutrition extends to any effect of food on human function: fetal health and development, resistance to infection, mental function and athletic performance. There is growing interaction between nutritional science and molecular biology which may help to explain the action of food components at the cellular level and the diversity of human biochemical responses.

Nutrition is also about why people choose to eat the foods they do, even if they have been advised that doing so may be unhealthy. The study of food habits thus overlapswith the social sciences of psychology, anthropology, sociology and economics. Dietetics and community nutrition are the application of nutritional knowledge to promote health andwellbeing. Dietitians advise people howto modify what they eat in order to maintain or restore optimal health, and to help in the treatment of disease. People expect to enjoy eating the foods that promote these things; and the production, preparation and distribution of foods provides many people with employment.

A healthy diet means different things to different people. Those concerned with children’s nutrition—parents, teachers and paediatricians—aim to promote healthy growth and development. For adults in affluent communities nutrition research has become focused on attaining optimal health and ‘preventing’—which mostly means delaying—chronic degenerative diseases of complex causation, especially obesity (Chapter 16), cardiovascular diseases (Chapter 18), cancer (Chapter 19) and diabetes (Chapter 20).

Have you ever stopped and wondered why we (humans) are as we are, and why we do what we do? It is truly remarkable what our bodies are capable of doing and how our bodies operate to perform various tasks. Yet, we are just one of millions of different species inhabiting this planet, all with a unique story to tell. And, like our fellow planet-mates, we must abide by the basic objectives of life, namely to function as an independent being (self-operate), defend ourselves both externally and internally, nourish ourselves, and of course to reproduce, which is without question the ultimate objective of all life-forms.

Yet, we are special in that we have a relatively large brain and the intellectual capacity to try to understand ourselves and, in accordance, how we are to be nourished. In this chapter we will begin to explore the very basis of our being and the world we live in. This will begin to set the stage for understanding what it will take to nourish our body for optimal health and longevity. We will answer questions about basic concepts such as elements, atoms, molecules, oxidation, chemical reactions, water solubility, and acids and bases. If you have a science background this chapter might seem too rudimentary and you might consider moving on to the next chapter.

What Is Nutrition?

We will start out as simply as possible. The shortest definition of nutrition is the science pertaining to the factors involved in nourishing our body. Nutrition hinges upon the special relationship that exists between our
body and the world we live in. From the moment of conception to the waning hours of advanced age, we live in a continuum to nourish our body. More specifically, we strive on a daily basis to bring nourishing substances into our body. These nourishing substances are called nutrients, which are chemicals that are used by our body for energy or other human processes. Proper nourishment supports body businesses such as
growth, movement, immunity, injury recovery, and disease prevention, and, of course, the ultimate business at hand for all life-forms, reproduction.

A healthy balanced diet is vital for good health both from the perspective of an elite athlete and for those who enjoy working out to keep fit. Training can be optimised to help athletes and exercisers to reach their goals by making informed dietary choices. When we were approached to write this handbook the original idea was to write a book for general practice. However, we all remember being student dietitians and all created our own handbook of useful information that we carried around with us and were totally lost without. On reflection of what text books are now available in nutrition or dietetics, it became clear that although there are now concise pocket books written for dietitians working predominantly in a clinical setting, there was a need for a user friendly handbook of nutrition and dietetics for a wider audience that included doctors, nurses, nutritionists and other health care professionals. The available textbooks are, by necessity, large tomes or series that are unlikely to adorn the shelves of many doctors or nurses whether in primary or secondary care.

As a result, we have tried to present nutritional science, therapeutics and community public health nutrition in a concise and integrated manner. While writing the text we have tried to identify what information would be useful to different professionals in a variety of settings. For example a doctor or nurse may want information on obesity and will find a ready reckoner for the calculation of BMI, information on associated problems and treatment options. Dietitians working in the community or public health will have this information but will find the sections on the measurements of obesity or nutrition interventions more informative. How well we have achieved this is for the reader to decide.

Nutrition is fascinating for many reasons, one of which is the fact that it is a very dynamic discipline. We have tried very hard to be contemporary but there will inevitably be changes in basic science, practice and policy as the discipline continues to evolve. Major developments and changes will be posted on the relevant page of the OUP web site. For us it has been a very enjoyable, if at times rather demanding, process and we hope that this book is useful to all health care professionals
